HCC Privacy Practice Information
Student records are covered by the Family Educational Rights and Privacy Act of 1974 (FERPA) and other privacy policies. Learn more about University of Denver privacy policies.
We understand that health information about you and your health care is personal. We are committed to protecting health information about you. We create a record of the care and services you receive from us. We need this record to provide you with quality care and to comply with certain legal requirements.
We are required by law to:
- make sure that health information that identifies you is kept private;
- give you notice of our legal duties and privacy practices with respect to health information about you; and
- follow the terms of the notice that are currently in effect.
We distribute our notice to you through the MyHealth portal and request it be acknowledged annually.

Request Your HCC Records online
The easiest way to request your records from the DU Health & Counseling Center is through the MyHealth portal. You can do so by following the steps below.
- Log in to the MyHealth portal
- Go to Messages
- Select New Message
- Select "I want to request a Release of Information" (second to last option)
- Follow the prompts
Authorization for Release of Information (ROI) Form
Alternatively, you may request your Health & Counseling Center records by completing and submitting an Authorization for Release of Information form. The University of Denver requires an actual signature on this form. Forms with an Adobe Digital ID will be rejected.
Please note that we do not process ROI’s for future appointments. If you would like information released after any of your appointments you will need to complete a ROI after each one.
If you wish to release your records to another area within DU, please complete and submit a Campus Partners Authorization for Release of Information form. The University of Denver requires an actual signature on this form. Forms with an Adobe Digital ID will be rejected.
Submitting Your Form
There are four ways you can submit your completed and signed Authorization for Release of Information form:
- Email it to hccinfo@du.edu
- Fax it to 303-871-4242
- Submit it in person at the Health & Counseling Center on the third floor of the Ritchie Center, north side.
- Mail it to the address below. Please allow extra processing time if submitting your form by mail.

Retention of Records
Unless otherwise required by law or policy, we maintain HCC records for 10 years from last HCC service date.
Mobile Privacy Policy
The University of Denver Health & Counseling Center conforms to the University of Denver Privacy Practices.
The University values constituents’ preferences in receiving information and messages. This includes messages to mobile devices through Text Messaging, such as the HCC's text appointment reminders feature.
The University will use Text Messaging to effectively communicate while being a good steward of information, respecting the privacy and wishes of the recipients, and adhering to applicable state and federal laws, including but not limited to the Telephone Consumer Protection Act.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes.
Artificial Intelligence (AI) Assisted Clinical Documentation
Overview: The DU Health & Counseling Center (HCC) uses approved Artificial Intelligence (AI) technology within the Point and Click Solutions (PNC) electronic health record platform to support clinical documentation and administrative workflows. AI is used only as a documentation assistance tool and does not replace the clinical judgment, expertise, or decision-making authority of licensed healthcare professionals.
How HCC Uses AI: When patients provide informed consent, approved AI tools may be used for transcription, draft visit notes, session summaries, referrals, after-visit instructions, and administrative documentation support. All AI-generated content is reviewed and approved by a provider before becoming part of the medical record.
AI Is Not Used For: Providing psychotherapy, making diagnoses, treatment decisions, emotion detection, or replacing professional clinical judgment.
AI Infrastructure and Data Flow: PNC AI operates in Microsoft Azure using a private PNC-managed model. Data is routed through the secure PNC AI Aggregator, encrypted using HTTPS/TLS, processed, and returned to the secure clinical documentation environment.
Privacy and Security Protections: PNC does not use customer data for model training. PHI is not shared with outside AI vendors. The platform operates under the same security controls as the broader PNC environment and aligns with SOC 2 Type II and HECVAT reviewed controls.
Microsoft Azure Protections: Data submitted through Azure OpenAI is not shared with other customers, is not available to OpenAI, and is not used to train foundation models or improve products without permission.
Patient Rights: Participation is voluntary. Written consent is required before AI transcription or summarization. Patients may decline or withdraw consent at any time without impacting access to care.
Regulatory Compliance: HCC AI use aligns with HIPAA, FERPA where applicable, the Colorado Privacy Act, Colorado State Law (eg, C.R.S. § 12-245-224.5 within Title 12, Psychotherapy Artificial Intelligence Restrictions), and University policies.
